Bereavement is deeply personal. It may be a common experience to all of humankind, but it is a unique, individual process—a path that each person discovers anew. It does not however, mean you have walk it alone.

To Help You On Your Journey...

Concerned Care Hospice provides bereavement services for 13 months following the death of a hospice patient.


A staff of trained professionals provides support to individuals and families that have lost a loved one who has been in our hospice care.


The Bereavement team consists of the Bereavement Coordinator, Chaplains, Social Workers and Volunteers.
